Catastrophic Program

A “catastrophic injury” is defined as an injury that has sudden life altering consequences, usually with sudden onset, and requiring extensive medical treatment and intervention. 

Encore Unlimited, LLC provides a comprehensive and innovative approach to successful catastrophic case management. We are committed and dedicated to ensuring that our catastrophic services are of the highest professional standards and quality.

Encore Unlimited's Catastrophic Program requires immediate dispatch to the medical facility to:

  • Immediately assess the medical situation of the patient

  • Provide support and information back to the family.

  • Provide continuous communication to the insurance company and the employer.

Encore Unlimited's Catastrophic (CAT) Referral Process:

  • Call 800-434-2520 to make a request for service during normal business hours of Monday through Friday 7:30am to 5:00pm CST.

  • Call 715-498-2951 for after hours requests (after normal business hours, holidays, and weekends).

  • Ask for the Referral Coordinator

  • Provide as much information as you have regarding the case. Most importantly how the nurse should reach YOU (the adjuster) via phone. We realize information is often sketchy during a time of crisis.

  • Our Referral Coordinator will dispatch the closest CAT nurse. That nurse will begin immediate case management activities, and will call the adjuster en-route to discuss the case. A formal initial evaluation with recommendations is emailed to the adjuster within 48 hours.
